    What is Chronic Kidney Disease?

    Chronic kidney disease (CKD) is a long-term condition in which the kidneys gradually lose their ability to filter waste and excess fluids from the blood. It affects approximately 37 million Americans — many of whom are unaware they have it, as CKD can progress silently for years with few or no symptoms in the early stages.

    CKD is classified into five stages based on the glomerular filtration rate (GFR), a measure of how well the kidneys are filtering. At Associated Kidney Specialists of the North Bay, we intervene early — even at Stage 1 and 2 — to identify root causes, slow progression, and prevent complications before they occur.

    The most common causes of CKD include diabetes mellitus, hypertension, and glomerulonephritis. However, genetic conditions, recurrent kidney infections, obstructive uropathy, and certain medications can also contribute. Our approach is to address both the underlying cause and the downstream effects of reduced kidney function simultaneously.

    Our Approach to CKD Management

    At AKSNB, our CKD management protocols are grounded in the most current KDIGO (Kidney Disease: Improving Global Outcomes) guidelines and incorporate the latest disease-modifying therapies. We don't simply monitor — we actively intervene to protect your kidneys and your overall health.

    Our nephrologists are experts in prescribing SGLT2 inhibitors (such as empagliflozin and dapagliflozin), which have demonstrated remarkable kidney-protective benefits beyond blood sugar control. We also optimize renin-angiotensin-aldosterone system (RAAS) blockade and finerenone therapy where appropriate, tailoring each medication regimen to your individual risk profile and tolerability.

    We work in close collaboration with your primary care physician, endocrinologist, cardiologist, and dietitian to ensure that every aspect of your care — from blood pressure targets to dietary phosphorus — is coordinated and optimized. This multi-disciplinary approach is essential because CKD rarely exists in isolation; it is intimately linked to cardiovascular disease, diabetes, anemia, and bone mineral disorders.

    For patients with advanced CKD (Stage 4 and 5), we proactively begin planning for potential kidney replacement therapy, including thorough pre-dialysis education and early transplant evaluation, so that patients are never caught unprepared when the time comes.

    Managing CKD Complications

    CKD is not just a kidney problem — it affects virtually every organ system in the body. Our comprehensive CKD care addresses all of the major complications that arise as kidney function declines:

    Anemia of CKD: As GFR declines, erythropoietin production falls, leading to anemia. We monitor hemoglobin levels regularly and prescribe erythropoiesis-stimulating agents (ESAs) and IV iron therapy when indicated, carefully balancing treatment benefits against risks.

    CKD-Mineral Bone Disorder (CKD-MBD): Impaired phosphorus excretion, vitamin D deficiency, and secondary hyperparathyroidism are hallmarks of advanced CKD and contribute to bone fragility and vascular calcification. We monitor and treat these with phosphate binders, active vitamin D, and calcimimetics as appropriate.

    Cardiovascular Risk: Patients with CKD have dramatically elevated cardiovascular risk. We prioritize lipid management, blood pressure optimization, and antiplatelet therapy coordination as part of every CKD patient's care plan.

    Metabolic Acidosis: Declining kidney function impairs acid excretion, leading to metabolic acidosis that accelerates muscle wasting and kidney progression. We monitor bicarbonate levels and prescribe oral bicarbonate supplementation to maintain appropriate acid-base balance.
