Hypertension and Kidney Disease: A Dangerous Cycle
Hypertension (high blood pressure) and kidney disease share a deeply intertwined relationship — each condition can cause and worsen the other. Chronically elevated blood pressure damages the delicate blood vessels within the kidneys, impairing their ability to filter waste. Conversely, when the kidneys are damaged, they often drive blood pressure higher by retaining excess salt and activating hormonal systems like the renin-angiotensin-aldosterone system (RAAS).
This bidirectional relationship makes hypertension management in the context of kidney disease uniquely challenging — and uniquely important. Achieving tight blood pressure control is one of the single most powerful interventions available to slow the progression of chronic kidney disease and reduce cardiovascular mortality.
At Associated Kidney Specialists of the North Bay, our nephrologists bring specialized expertise to the evaluation and treatment of hypertension across its full spectrum — from straightforward primary (essential) hypertension to resistant, refractory, and secondary forms that require more sophisticated investigation.
Secondary Hypertension: Finding the Hidden Cause
In most people, high blood pressure has no single identifiable cause (primary or essential hypertension). However, in a significant minority of patients — particularly those with difficult-to-control blood pressure, young onset, or associated symptoms — there is an underlying secondary cause that must be identified and treated.
Renovascular hypertension (caused by renal artery stenosis) is a common secondary cause that our nephrologists evaluate using Doppler ultrasound and CT angiography. We collaborate with vascular surgery and interventional radiology for revascularization when appropriate.
Primary aldosteronism (adrenal overproduction of aldosterone) is now recognized as the most common identifiable secondary cause of hypertension. We screen appropriate patients with plasma aldosterone-to-renin ratios and coordinate adrenal vein sampling for definitive diagnosis.
Other secondary causes we investigate include pheochromocytoma, Cushing syndrome, sleep apnea, thyroid disorders, and coarctation of the aorta. A thorough evaluation saves patients from years of inadequate blood pressure control and its consequences.
Resistant and Refractory Hypertension Management
Resistant hypertension — blood pressure that remains above goal despite the optimal use of three or more antihypertensive agents — affects millions of Americans and carries a dramatically elevated risk of heart attack, stroke, and kidney failure. True resistant hypertension (distinct from pseudo-resistance due to white-coat effect or medication non-adherence) requires expert nephrology evaluation.
Our approach to resistant hypertension includes comprehensive ambulatory blood pressure monitoring (ABPM) to confirm true 24-hour hypertension, systematic evaluation for secondary causes, optimization of diuretic therapy (including the use of mineralocorticoid receptor antagonists), and careful medication titration to achieve target blood pressure while minimizing side effects.
We also evaluate patients for newer interventional options where appropriate and maintain current knowledge of evolving pharmacological therapies for complex hypertension management.
